- Pay the sum of N10,000 (Ten Thousand Naira Only) into NIM Account number:2000206978 with the First Bank of Nigeria Plc.
The Teller must be written in YOUR NAME please.
- Use your teller to fill your information on the FORM online. Ensure your NAMES (surname first) are typed exactly as you want them printed on your CERTIFICATE.
They should be in CAPITAL LETTERS AND WITHOUT ABBREVATION
- USE CAPITAL LETTER while filling the FORM ONLINE
- Your dates of Payment and BIRTH must follow the pattern YYYY-MM-DD e.g. 2008-01-24
- Where a required information is not applicable PLEASE put NONE
- DO NOT LEAVE ANY BOX EMPTY
- Click SUBMIT DATA to send you details to our DATABASE. But do not click more than ONCE
- Please ensure the information supplied are correct and nothing more.
The Institute will not be liable for any error or mistake in the information herein supplied.
- You are expected to select a CHAPTER when filling the form. A CHAPTER of the Institute is a GROUP OF MEMBERS in a particular geographical location,
e.g. town, city, local government, Institution, etc, as the case may be. You are expected to be identified with the Institute at the CHAPTER Level.
Please, pick a CHAPTER of your choice depending on where you are living or working.
You could change your CHAPTER at any time in case of relocation or job transfer.
